If you or your loved one live in Palatine Township (Illinois) or surrounding area, contact the Social Services Department of PTSCC (847-991-1112) for help with the following services and/or referrals to other local providers:
 

  • Adult Day Program - Caring Hearts Program
    A program for those with memory loss that gives those caring for them a break during the day.  Offered five days per week, four hours per day, this program offers exercise, socialization, activities to help with daily living skills, and lunch.  Participants must be continent and able to feed themselves.  Space limited to 10 participants.

     
  • Benefits Eligibility Screening – Our Social Services Staff can give one-on-one assistance to help you find find out for what benefits you may be eligible to receive.

  • Bilingual/bicultural outreach

  • Caregiver Support Services
     
  • Carrier Alert Program – Do you worry that you may have an accident in your home and no one will know about it?  If you live in the service area of the Palatine Post Office, you can register to participate in Carrier Alert.  If your letter carrier notices uncollected mail, follow up calls will be made to ensure your safety.  Free.
     
  • Case Advocacy and Support – Services to assist an older adult or their family
     
  • Counseling – Bereavement, Individual and Family
     
  • Information and Referral to additional resources.  PTSCC works with many other agencies to coordinate services and benefits to meet the individual older adult’s needs.
     
  • Meals on Wheels (Home Delivered Meals) Assessments
     
  • Multicultural Program

Our Volunteers provide the following services under the Agency’s supervision: 

  • Escort/transportation to medical appointments
     
  • Friendly Visitor Program for homebound older adults to alleviate loneliness.
     
  • Medicare/Supplemental Insurance Assistance Trained counselors provide helping sorting through the red tape associated with Medicare and other health insurance
     
  • Shopping Assistance
     
  • Yard Work/Home Repair  (in conjunction with projects such as Rebuilding Together and the Paint-a-Thon)
